Documents must be submitted in portable document format (PDF). PDF Portfolios (i.e. a package of multiple PDF documents) will not be accepted. The following documents must be merged into one PDF document for upload: 1) Nomination letter; 2) Description of team’s joint contributions; 3) List of potential External Reviewers; 4) Letter identifying the spokesperson; 5) Indentification of any eligible leaves of absence (optional); 6) Signed Terms and Conditions Form for each nominee; and 7) Signed Terms and Conditions Form for each Nominator.
