Documents must be submitted in portable document format (PDF). PDF Portfolios (i.e. a package of multiple PDF documents) will not be accepted. The following documents must be merged into one PDF document for upload: 1) Nomination letter; 2) Documents relating to the advance; 3) List of potential External Reviewers; 4) Letter identifying the spokesperson (for team nominations only); 5) Identification of eligible leaves of absence (optional); 6) Signed Terms and Conditions Form for Nominees; and 7) Signed Terms and Conditions Form for Nominator.
