Documents must be submitted in portable document format (PDF). PDF Portfolios (i.e. a package of multiple PDF documents) will not be accepted. The following documents must be merged into one PDF document for upload: 1) Nomination letter; 2) Description of each nominee’s involvement in research and/or knowledge mobilization; 3) Documents providing evidence of knowledge mobilization and societal impact, 4) List of potential reviewers; 5) Letter identifying the spokesperson (for team nominations only); 6) Identification of eligible leaves of absence (optional); 7) Signed Terms and Conditions Form for each Nominee; and 8) Signed Terms and Conditions Form for each Nominator.
